Community Service A. Letters form a community organization(s)- Two (2) outlining the type of volunteer service performed and time frame involvment
Letters or Recommendations- One(1) B. Teacher and/or School Counselor
Achievements and Awards A. Short paragraph of goals and objectives during college and after college (minimum 600 word essay). B. List awards: Organizations; Dates; Services provided; C. List activities during school life
A small photograph is required.
